President Donald Trump is expected to maintain the Medicare drug price negotiation process, but with potential modifications. These changes could impact the 68 million Medicare beneficiaries and major pharmaceutical companies like Novo Nordisk, Bristol Myers Squibb, Pfizer, and Merck. The Inflation Reduction Act (IRA) allows Medicare to negotiate prices for drugs on the market for at least seven years without generic competitors, or 11 years for biologics. Trump might adjust how drugs are selected for negotiation, possibly benefiting drugmakers by lessening revenue loss from lower prices. The administration could also alter the negotiation process to include more stakeholders like insurers and pharmacy benefit managers. The next cycle of negotiations, involving 15 drugs, will see new prices in 2027. Trump’s approach to ongoing legal challenges by the pharmaceutical industry remains uncertain, with nine lawsuits currently active.
